Diplomates and applicants may view and update addresses, telephone numbers, and e-mail address. Security is maintained by requiring your last name and your password or Social Security Number to gain entry into the update page. This section of the web site is protected by SSL encryption. In some cases the Board does not have Social Security Number information. If you are not able to gain access to your demographic information please contact the Board office at (312) 939-2276.

You are not able to change your name or educational degrees on the web site.  Since the information provided here is used as primary source verification by credentialing organizations, the Board must verify changes to name and post-graduate educational degrees directly from primary sources. To make name changes or to add post-graduate degrees not listed on this web site please contact the Board office at (312) 939-2276.

We strongly encourage all Diplomates and Applicants to maintain current contact information so we can keep you informed of the latest information from your Board.

Name, address, identification, and certification information is provided to the American Board of Medical Specialties (ABMS) as is required for all Member Boards of the ABMS.

Mailing address information is provided to the Aerospace Medical Association, the American College of Preventive Medicine, and the American College of Occupational and Environmental Medicine. The database of diplomates certified by the American Board of Preventive Medicine is not provided to any other organizations nor sold to any third parties.
