Medical Toxicology Eligibility Requirements

	ACGME-Accredited Fellowship Pathway
Pathway Purpose	This pathway is intended for physicians who have completed a 24-month (or longer) ACGME-accredited Medical Toxicology fellowship program.
Board Certification	ABPM specialty certification from ABPM (Aerospace Medicine, Occupational and Environmental Medicine, or Public Health and General Preventive Medicine) Physicians certified by another ABMS member board should apply through the American Board of Emergency Medicine.
Fellowship	Completion of a 24-month (or longer) ACGME-accredited Medical Toxicology fellowship program

Documents and Verifications

Applicants should gather the following documents and verifications when they apply.

CV/Resume	Required if applying 24 months after completing fellowship
Board Certification Verification	Required; ABPM staff will verify current primary specialty certification.
Fellowship Verification	Applicants must provide fellowship program information. ABPM will independently verify fellowship completion with the program director.
Letters of Reference	One letter required if applying 24 months after completing fellowship
