Applicants should gather the following documents and verifications when they apply.

| Letters of Reference | One letter required only if applying between 24 months and seven years after completing fellowship.1 The letter must be written by a physician who is certified by an ABMS member board, the AOA, the RCPSC, or the CFPC, and who can attest to the applicant’s experience in Addiction Medicine. |
