Addiction Medicine Eligibility Requirements

	ACGME-Accredited Fellowship Pathway
Pathway Purpose	This pathway is open to physicians who have completed a 12-month (or longer) ACGME-accredited Addiction Medicine fellowship program.
Board Certification	Primary specialty certification through an ABMS Member Board, the American Osteopathic Association (AOA), the Royal College of Physicians and Surgeons of Canada (RCPSC), or the College of Family Physicians of Canada (CFPC)
Practice Activity	N/A
Fellowship	Completion of a 12-month (or longer) ACGME-accredited Addiction Medicine fellowship program

Documents and Verifications

Applicants should gather the following documents and verifications when they apply.

CV/Resume	Required only if applying between 24 months and seven years after completing fellowship.1
Board Certification Verification	Primary specialty certification through an ABMS Member Board, the American Osteopathic Association (AOA), the Royal College of Physicians and Surgeons of Canada (RCPSC), or the College of Family Physicians of Canada (CFPC) is required.
Practice Verification	N/A
Fellowship Verification	Required
Letters of Reference	One letter required only if applying between 24 months and seven years after completing fellowship.1 The letter must be written by a physician who is certified by an ABMS member board, the AOA, the RCPSC, or the CFPC, and who can attest to the applicant’s experience in Addiction Medicine.
